Default Issues With My Geriatric Little Dexter

Hello, my little Dexter is a 15 year old Yorkie who has lost his vision, and hearing. He has had incontinence problems for some time now. We are constantly cleaning up messes, even on our bed. A couple of days ago, he stopped eating and was drinking very, very little. He's lost another pound (over the last 1.5 years). He now weighs 3.3 pounds.
I decided to make him some food, ground beef with sweet potato and green beans which we mashed up. He loved it and also began drinking water. Whew!
Last night, he woke up and stayed basically in a panic for most of the night. We took him outside and he trotted in the yard aimlessly for an hour, he is now doing the same thing inside. If he barely bumps into something, he cries. He is whining silently, constantly. He won't let us hold him for more than seconds. He just constantly wanders. Does anyone else have any experience with this type of behavior?
I'm worried.
