Default Aren't shots required in order to get their heart worm meds?

I'm so confused about shots. I have a four-year-old Yorkie named Izzy. I have changed vets three times due to the outrageous costs for her shots every six months. The newest vet only gives them once a year which helps, but the first time I took her in the vet gave her all the shots at once. Izzy was so sick. She didn't throw up, but she was so lethargic for two days that it scared me to death. I've never seen her that way.

I give her Triflexis for heart worms, which is prescription. Can anyone tell me what is the least amount of shots I can give Izzy to keep her protected and still be able to get her heart worm pills?
