I think the only way to know how much the monthly quote would be is to request an estimate from each individual insurance company you are thinking about using. I would think that each pet insurance company quotes different prices just like different home, car and health insurance companies do plus it also depends on the age and the health of the dog.
When I was looking for pet insurance, I narrowed it down to four that I was interested in and then completed each company's questionnaire for an estimate.
