This is a complicated issue bc the dog could be doing this for various reasons. If this doesn't resolve / get better within a couple/few weeks..then honestly, you really do need to have a good trainer come to your house, watch the behaviors, and guide you in a plan to make this better. I know it'll be a bit of an expense, but it should work if you have a good trainer.
I'd hate to see this poor little doggie shuffled yet again to another home, so I do hope you find something that works.
