Is some of anxiety related to a bit of age-related dementia / confusion setting in for her already? If so, I can then see why/how she may be going potty suddenly in this new habit. For pads, you could try getting some of her urine on the actual pad so that she can smell it and perhaps start associating that smell w/ where to go.
