It's 3 times the cost to register an unaltered dog, so when people register, they say their dog is altered even when it's not. No one checks. There's no medical proof necessary, it's just how you fill out the form.
When you register a dog here where I live and you check off altered or not, you have to send in proof from the vet with the application form. I found that out when I went to register my two puppies. One had been neutered, the other was too young yet to be spayed so I only registered the older one and am waiting until I get my younger one spayed to be registered.
