he said we could take him off and see ... he has not concrete diagnosis since we could not do the spinal tap and we dont think he should go under gain to do it just to get a diagnosis ...

I would not take him off his meds! You could do a consult with a vet nutritionist who could formulate a diet that would satisfy him but not put weight on him. I have found that the pups I home cook for get more food than when I fed them dog food.
