I have this for mine! My name and phone number on it! I found a Yorkie running down the street one night when I was coming home from picking my son up from work. The next day, I called the vet on the tag....they looked up the rabies # and said that woman had moved and did not come to that vet office any more. GREAT!! I asked if they had forwarded records to any other vet...."no". Fortunately, the NEXT day, there were signs and posters all along the street, on all the traffic signs, etc....I called the people so the story had a great ending for little "Joy-Joy".....if they would have had a embossed collar with the owners name and phone number embroidered on it, I could have called them as soon as I got home with their baby girl.
