Usually the difference is a reputable, respected breeder with years of hard-earned knowledge in genetics/breeding/pregnancy/whelping/puppycare and development who ensures health certifications on the breeding dogs in or out of her operation, breeds solely to improve the breed vs. a back-yard or hobby breeder who breeds any two dogs together to produce a litter, usually the smaller the better for faster, larger dollar sales.
