Has the vet done a skin slide (cytology) ? I had a dog with thinning hair and it was a bacterial infection. We put him on simplicef (antibiotics) for a month and used medicated shampoo (malaseb) and it really helped. Maybe your vet will go for that idea since he's stumped.
