My older male used to be sneaky with marking. Also when I wasn't looking but in more "hidden" places rather than in plain sight. He would mark in the bedroom a couple drips on the bed skirt or on the curtains in the living room. If the medical issue is ruled out you may have to get belly bands. That helped with mine bc once he realized he wasn't able to actually mark his territory, he stopped needing the belly bands. It's years later now and I can honestly say he only marks rarely now. Maybe once or twice a year. So it really helped my situation. He was my only dog at the time and I also got rid of the peep pads and he was going only outside. That may have helped too.
