You might have your vet take a look at him. I think another poster here has talked about a condition where their boys penis was stuck "out" and was red and inflamed and they'd have to use vaseline on him to tuck it away.

It can get infected like you're concerned about and is extremely painful for the little guy, so keep an eye on it, keep him from licking it and if you see it poking out again, make your vet check it out.
