Sometimes what is helpful is to videotape the coughing. There is kennel cough, reverse sneezing, and other reasons for coughing. Has he done the whole course of treatment with the antibiotic?
On this site there is a stickey in the library that has a pretty good video of three of the different coughing sounds.

I am sure your vet has told you this; but at 1lb you do need to be cautious about hypoglycemia. If you are un-certain about this let me know and I can direct you to some good articles as well :-)
