Sad Please help!

Hi Everyone,
Our puppy Boss has been home with us now three weeks. He's great, in perfect health and very active. We took him home at 14 weeks and he had been trained slightly at the breeder to use pads. At home we are using pads until it gets warmer out. The past three weeks have gone extremely well. I could count on one hand how many accidents we had. most of the time he walks over to the pads on his own when he has to go and at night wakes me when he needs to do his stuff and then right back to bed. PERFECT. Then yesterday we had the weirdest day. He peed at least eight times, and six were off the pad. He was right next to it but went anywhere but there. Then, before bed after he finished going to the bathroom he was playing on our bed while we were reading and without warning peed right on it.
This is where I need help. Is this just a usual slip up in potty training or are we to start from scratch? Today was a good day and then a few minutes ago he peed on the couch, no warning. I spoke with the vet (we were there today to get a shot) and she said he was fine. Anyone come accross an incident like this? I was bragging to everyone how good we were doing! Any suggestions or ideas would be incredibly appreciated from all the expreinced yorkie mommies or daddies out there.
Thanks in advance!
