Is she peeing and pooping outside the fence of the xpen where the pads are? Does she have to hop over the little gate area to get inside? Maybe she is reluctant to do that when she has to go potty.
You are doing everything correctly: limiting freedom to the smallish area with the pads easily accessible.
You can also try changing the pads often. When Max was a pup, he didn't want to go near a used pad.
When she gets it right -- 100% on the pad, praise her profusely and give her a small treat. When she gets it wrong, say "Uh oh, missed the pad." Then be sure to clean the spot thoroughly so the scent doesn't attract her to return to that spot.
You can put a piece of linoleum down under the pads to protect your carpet and made cleanup easier.
