For your trip, I'd get some sort of a car seat/snoozer seat that you can either put on your middle console and/or in the passenger seat. Bring extra bedding/blankies to make it nice and cushy and secure for your little one. Being so young, most likely she will sleep most of the time.
Bring along baby wipes, pee pads, towels, water/water bowl, treats, her regular food she is on now, Nutrical (!!), an empty syringe to force water if ever needed...and I can't think of what else....?
Also, your breeder should know whether her puppies are prone to hypoglycemia, but even if they're not, still bring along the Nutrical.
