When he naps during the day or night I'll take him out immediately after waking, and he goes potty outside every time I take him out (about 90% of the time), but he can't/won't hold it longer than even an hour anymore.
The thought of crate training again from day 1 with 75% of his day in the crate terrifies me as I know how awful he is the crate when he's awake and it bothers him, me, and everyone in the house. It sounds like someone is literally killing him.
Does anyone have any ideas, or maybe something I'm missing? I wondered about a UTI, but he seems to be in perfect health, no loss of appetite or energy, no blood or crystals in the urine, and it's not only pee that I'm having the problem with.

I don't think I can give you much advice except for this: have you asked your vet? Maybe he/she will be able to determine the problem.
If he whines/screams when he is in his crate: does he do this ALL of the time, or only for extended periods? For this, I would use an X-pen--gives him more room, while still being confined.
