It sounds like you need to start over with a new plan. Confine her to small area (gates or an expen) put down a wee pad and when she uses it give her a treat. You need to clean up the smell of where she has had accidents and limit her freedom until she gets the hang of this. keep her near you (on a leash) and take her to the pad or outside often.
