Sorry to hear about Sophie, but it's a good thing she has you to help her through this.

I haven't been through this with one of mine, but several here have and hopefully they will chime in soon with some advice.

Typically they are put on a prescription food that you will get from the vet, but home cooking is possible IF you get a consult with a vet nutritionist to get a properly balanced recipe that takes her special needs into account.

Did your vet mention any more tests? If it truly is portosystemic shunt, your Sophie will need surgery and they typically do very well after that. If it isn't one big shunt but many microscopic ones (MVD), that can't be fixed with surgery but they generally do well as long as they are on a special diet and take supplements.
