Yes, I'd get him to a vet IMMEDIATELY. This could be SO many things, some of them life threatening. Most concerning of all would be parvo, which can be deadly in puppies. Parvo that is progressing would come w/ watery diarrhea and vomiting. Truly, you NEED to get to a vet ASAP.
