Question Separation. Anxiety

Does anybody have tips for helping Toni cope with occasionally being away from me? The vet has prescribed Tramadol with no success.
I have tried going outside while being visible to her still she screams until I am indoors with her. We have repeated this exercise many times with no improvement.
When I do have to leave her she is safe in her crate with something soft to sleep on, toys, something with my scent, treats and a pee pad. I put something on top of it to make it feel more like a den
When I get home she has gotten so upset that she vomits and the pee pad is shredded, the crate is trashed. It takes at least ten minutes to clean up the mess. She is not punished, I hold her and tell her I will always come back to her.
This behavior lasts several more hours, the vet said she is not acting out just trying to release some of the anxiety. My heart is broken. I hate being away from her too, Toni is my therapy dog and goes most places with me.
What can I do to make the separation easier for us both?
