Default She escaped!

We have been putting Lexi in the kitchen blocked off by two sets of gates. The gates don't lock onto the wall, but are pretty heavy so she is not really able to knock them down and they are about 3 feet high. Today after getting back from grocery shopping she was no longer in the kitchen! I really don't know for sure how she got out. One of the gates was slightly moved, but no gap for her to really get through so I think she could have climbed over? Not sure. Anyway, now I don't know what to do. I'm worried she will try to get out again and hurt herself, but I'm also worried about her hurting herself if I leave her alone in the whole house. The biggest issue is she sometimes jumps up the back of the couch that is up against a half wall (just a foot higher than the couch) and on the other side of the half wall is our dining room with tile floors. I have been trying to teach her not to jump up there because if she jumped up and fell on the other side she would definitely hurt herself. Usually she does it when I'm in the dining room and she wants to see what I'm doing to get my attention. Not sure what I should do now. Any advice???
