I read your post, and was not clear if the vet knew of the symptoms and declined to comment, or did not know of his symptoms, and just pronounces him well on his vet visits?
But I will say constant or frequent twitching shaking is not normal. There could be a whole host of things that it could be, but without some diagnostics all we could do is just guess.
I believe for me, I would make sure the vet knows of his symptons and also that they appear to be worsening, and then see what he or she advises.
A referral to an appropriate specialist would not be out of line either.
