Could have been due to a nick, but also could be a hot spot or pyoderma (staph intermedius infection of the skin)...really hard to say. I'd normally say put some hydrocortisone cream and antibiotic cream on it - but if he's licking that heavily, I probably wouldn't. Some say that coconut oil helps w/ inflammation, so you could try putting some of that on (it's safe) until you make it to the vet.
