Abdominal distension can be indicative of some very serious things; he's also "posturing" his back to get relief. You do truly need to head back to the vet to get answers. This isn't normal at all, and I know you know this already - I'm just reiterating/confirming what you already know.

Did you change anything in his diet?
Is he known for eating things outside that he shouldn't, things that could cause blockage?
