The microchip needs to be registered only once, for the company that manufactured it. The vet will give you the necessary papers if they don't file the papers themselves. Then the chip number will automatically go into a national database and be accessible by vets, humane shelters, and animal control agencies who find lost dogs. The annual fees you hear about are for extra "bells and whistles" that are not necessary.
