It sounds to me like the employees were afraid of the man and while dementia patients can sometimes become violent it is not something that generally happens in the early stages.

This man was thinking very well and calculating how he could make some extra money and maybe make his practice easier and more profitable for himself. That is not how dementia manifests in a person.

A dementia patient would not have been able to continue to do surgeries and other medical procedures. His own dog had been suffering for a long period of time. This guy, to me anyway, has a mental disorder alright but he sounds more like some sort of sociopath. They can function "normally" in society but are lacking in the most basic of human empathy for anyone that may be suffering at any level. They are also very manipulative and can hid their disorder from people that are not close to them.
