To me it looks like she hasn't any idea that there is a proper place to go potty. If I were you, I would start back at step one with crate training. There is a wonderful book that I used called How to Potty Train Your Puppy in 7 Days. It has a kindle version from Amazon so you can download it immediately. It takes longer than 7 days, but the most important issue is that your puppy has to earn the right to run around the house.

I got a play pen to put my 2 in because my older girl was 6 (from a shelter, a rescue, had lived her entire life in a cage) and I didn't want to cause her anxiety by caging her. I got her last May and Barney, my other yorkie was just a few months old. For 2 days, I followed them around and kept a journal of every time they ate, drank, peed, pooped. I wrote down when, where, how much and what they were doing right before. Then, I made a schedule for them to follow using the information from the journal. Anytime I had to leave them, even if I had to go to the bathroom, they went in the pen. My husband made fun of me until we all went on vacation. The babies were used to pen time so they were fine if we went to the beach for a few hours while they were in the play pen.
Make sure to get lots of treats and be ready to praise her when she goes in the appropriate place. Anna just loves it. She will dance when my husband and I say good girl. Also, make sure to be right there when it happens. Don't just put her outside and assume she's making magic. Good luck with your training.
