You said he had red skin in his ears and under his legs and he was scratching. I'm very surprised the vet didn't suggest in the very least Benadryl and possibly a steroid, suggest a food changed or all of the above. I really think you need to either go back to your vet for reevaluation or move onto a Vet Dermatologist for diagnosis. I know that I wouldn't be very comfortable or happy it hung for weeks.
