Since we don't know what triggers it, the muzzle is a must to prevent injury. One example of an attack: during the night- Zoe woke up and threw up- Bella attacked her.
Another example: Sadie can wag her tail (to play) and Bella attacks. There's no time to say anything because it all happens so fast.
