When Max was neutered and had surgery, we received an itemized statement. Anesthesia, pain injection, and even hazardous waste disposal were each listed separately.

I don't think this is a matter of the vet being greedy. I think this is the vet giving owners the option of what they want for their pet. It shouldn't be optional, in my opinion.
