The longer the nails grow the farther down the quick will go but if the nails are just trimmed more often the quick will slowly work its way back down at least that what I have been told. Cutting them in the quick hurts just like it would hurt a human and it can also make them scared to get the nails cut in the future. I personally would never go back to that vet he obviously doesn't care about when Milo is feeling pain and that is scary.
