When he coughed, maybe he was throwing up a little and that could have created the white foam.

Being lethargic is worrisome. Are his gums pink and moist? If they are sticky and pale, or grey/white, that is more cause for concern.

Have you tried applying for Care Credit? They can tell you over the phone within minutes if you are approved. Then you have to check with the vet's office if they accept it.
