Definitely call the vet and bring him in. I am pet sitting a much older 22 months or so Yorkie, and she is cowering and shaking. But it has been now one month, and she is slowly changing. She has accepted pets, and jumped up on my lap. She is even getting brave enough to bark at my big boy.
It is just so very different. You might also try treating him with you on the floor.
