I have a 9 year old pet yorkie with a grade 3 and a grade 2 luxating patella, plus has hip dysplasia in one hip. Fortunately she has never limped or acted like she is in pain. If so, I'd have done surgery, but I too don't recommend surgery unless your dog is in pain currently, limping, holding leg up, etc. The surgery itself can still result in arthritis so you've got to weigh the good vs bad. Definitely consult with an orthopedic specialist if its something you're considering.
What grade did your vet say your dog has? Luxating patellas are so common In toy breeds that's its hard to completely rid your lines of the Problem. I personally have my dogs patellas OFA certified but know its hard to get all perfect patellas all the time. I do guarantee against high-grade luxating patellas requiring surgery but not all breeders do. If a breeder is producing a lot of grade 3 and 4s then they definitely need to re-evaluate their breeding program in my opinion.
