I have never been advised by a vet to do such a treatment...she would ask me to come in. If your vet did not see fit to advise you to bring him in, then you need to find another vet in my opinion!
I would do that now...he is in pain and could end up with an infection if not handled properly.
You need to keep those paws clean and yes, look for any debris...but personally, I would be at the vet. The paws are slower to heal on dogs because they must walk on them. He probably should have clean socks on...or be wrapped.
