No, I referred to the ICD9 book a lot...lol especially when someone had a broken finger or toe I could never remember all the digits. It is medicade and medicare... wow they were the worst for not excepting codes, such as sprains/pains and annual physicals.. Medicare will only allow a physical every 3 years.
I worked in two practices that had several doctors and nurse practioners and some of them would only use their little type recorders and I would have to wait for the charts to come out of dictation to bill the insurance companies. The doctors don't keep up with insurance rules, that is what they paid me for, (that is what they told me ...lol) so they had no clue that medicare wouldn't except sprain/pain, ect. And how many times would I have to refresh their memories..

But really we still had fun. I did work for some wonderful doctors.
This to was several years ago. Before most practices went to computers. It is proably much easier now...
