I worked with it from the insurance side many years ago.
I know insurance has changed a lot since then, but back when I worked with it everything was billed by ICD9 codes. ICD9s were a list of numbers that corresponded with each ailment/treatment.
Each ailment/treatment/number had a R&C (reasonable and customary) charge and that is what was billed. Back then doctors and insurance accepted assignment which meant that they would agree to the R&C and the designated charge would be accepted as payment in full.
This day and age its probably different.
I think, though, that from the billing aspect everything is probably automated and you would just need to become familiar with the ICD9s and the general procedures of whoever you would be working for.
Maybe do a google with ICD9 or medical billing and see if you can get mor current info.
