Default Why is potty training so hard for her

First off, hello! I am new here and it really looks like I found the perfect place to ensure that I am raising my baby girl Ruby Sue in the best Yorkie way possible :-)

Ruby is a 7 month old "Teaspoon" Yorkie. She is especially tiny (2.4 lbs) and has some health issues but none that should affect her ability to learn to potty train.

Ruby has been staying home, in a large play pen with a puppy pad while I work. I run home on lunch every day and take her out. We did try crate training but she was pooping and peeing in her crate. She would actually hold it while outside and then go inside and immediately poop and pee in the crate. I've "waited it out" with her outside for almost 2 hours, but now being in Colorado, it's getting too cold and she shivers and shakes and cries to go inside. She won't even go on her puppy pads. I also have a 5 year old teacup Chihuahua who is very well trained and was so easy. Ruby Sue is the biggest challenge of any dog I have ever owned.

FYI - I bought her because she had health issues and I wanted to ensure she would get the care she needed and deserved. She's under close watch by the vet by they have assured me that this is purely an issue of stubbornness. Any helpful suggestions I will greatly appreciate! Should I try a crate again? Will that make her feel too enclosed for the 4 hours that she is alone?
