Yes it pulls over the head. It has buttons where it would be on the back of the baby but it doesn't affect how it goes on. A lot of her stuff I do have to get taken up and this dress does need alterations. I have found though that all the stuff fits different. Some of the preemie stuff fits smaller than others. And most need alterations but other than that it works!
