This doesn't sound like worms to me ....the breeder I got my Chanel did the same thing and she had coccidia .....so ....this could something more...

If I were you - I would go BACK to the vet as soon as possible - I don't want to scare you but a very young puppy can go down FAST and this needs to be treated.

also keep him hydrated till you can see the vet....if he's not drinking try a syringe....
