If you find no medical problem, the only thing I can think of, is maybe she heard a loud noise or something may have scared her, while she was going ? Maybe try placing it in a different area of the house, where its real quiet? I've had to do that with my cat box before, because I figured out the neighbor had gotten a new dog, and because their box was below a window, one of my cats got scared of that area :-) if ll checks out good t the vet, I would consider trying something like this, you never know, our fur babies are very sensitive and have much better senses then we do :-) hope it all works out and you are able to figure out what is going on xoxo
