I've not done this for the described purpose, but...
I've made a "U" shaped piece of material, snugly wide where the D Ring is held well in place, and sewn the U to my Field Jacket, encapsulating the D Ring in the bottom of the U. If the width of the material is sufficient, the Ring will stay in place and not turn.
It was sorta like making a wide Belt Loop, and folding it in half to cradle the Ring.
