The main things I do are

(1) Command her to "Go Pee" when we're outside. She'll usually start sniffing for a spot right after. The "Go Pee" command should also be given when she is squatting and in the process of peeing or pooping outside (at least according to my yorkie book).
(2) Enormous praise every time she pees or poops outside. Every outdoor potty is an event that gets celebrated both when she does it and then again when she comes back in. Kisses and indoor playtime unless she goes off into the crate or asks onto the couch for a nap.
(3) After she goes it's outdoor playtime, unless it's the really hot part of the day when we just want to go back in lol.
(4) Stern NO when she squats indoors if I catch her in the act and immediate trip outside if she sniffs the floor or circles (this made potty training really hard the first 3 weeks or so though, because Cookie constantly had her nose to the ground both inside and outside then). Of course an outdoor trip if she actually pees inside too (luckily she hasn't pooped inside but maybe 3-4 times total, and all were at least 3 weeks ago).

We're still REALLY far from the goal, but I think this has gotten us on the right path.
