When I got my dog microchipped in 2009 I think, we didn't payy extra but we did register were given 2 places to register at. One was a mail in form from Avid and the other did online at Found Animals. I think Avid has a one time fee though.
Now at the same place with Draco it was just Found Animals, and was online registry and free.
Both have sites though that can help register and find places to get your pet microchipped.
