If her teeth are clean but she still has bad breath, it can be some other problem, acid reflux for example. Like others have mentioned, it can be a bad tooth or something you can't see. I never liked those water additives...how can something they drink help prevent plaque? If she doesn't let you brush her teeth, maybe using those dental wipes or even a gauze wrapped around your finger might help.
