Just a few thoughts.
Make the trips short and to fun places like a walk or a pet store, someplace fun not just groomers and vets.
check into a anxiety wrap for dogs. If I could post pics I show you how to do an easy one. It helps contain the axeity and it works.
Try some Tellington touch on the jaw line and teeth as well as the ears to get the good hormones the ones that calm flowing.
try just playing in and around the car not going any place and make it a ton of fum and treats a plenty.
