I'm not a vet, nor am I dog expert of any kind, but I think she might be confused. First she was pottying on pee pads, then she had a diaper on and could soil it whenever she had to go, knowing you'd change it. Now she's being crated plus taken outside for potty. That's three distinctly different potty routines plus the addition of a crate - so I think she's a bit confuzzled. How old is she?

I think you should decide which potty surface you want her to go on (pee pad, diaper, grass or patch) and then stick with that even if she has accidents (like not wanting the pee pad all of a sudden). She might be picky and wants a new one for every pee. Bella was fine with 4 pees before she wanted a pad change, but now that she's older she refuses to go unless I give her a new pad...which is fine with me, I don't like to pee in a toilet that has waste in it either.

Since you started with potty pads I would go back to those and start from scratch (like you just brought her home).

Consistency is extremely important and I think trying all these different things is confusing her.
