This is short, to the point and has held up in court for us.
God Bless You and congrats on Lily | Yers Teresa this is good. It's almost like what I had the previous owner of Luci sign when I took her in as re-home. I also specified on the form the breed of dog as well. I made two copies and we each signed in front of each other.
