I'd do what Cathy said.
The exam, blood, and fecal should be done on the same day because 1) it makes it so much easier to schedule yearlies, and 2) if vet sends to an outside lab getting more than one thing at once sometimes ends up costing less.

If you are looking to be very thorough, add on a u/a.
I'd do Lyme or Lyme and bordatella that day and lepto or lepto and bordatella a couple weeks later. The only bordatella I would consider is injectile (assuming the first was given intranasally).
Alternatively, you could give them all at once. It may not make that much difference.
It doesn't matter what we think about the vaccines you give. It only matters what you, your vet, the AAHA, and veterinary immunoloists think.
